On-Campus Office Information Change Request Form

For SHRA (SHRA) permanent and EHRA (EHRA) Non-Faculty employees use only. This online web form is used to change the employee’s on-campus office address and/or telephone information. All other address or contact information changes should be managed through UNCGenie. Once submitted, this form is automatically routed to Human Resources for processing. Please allow up to 48 business hours for the requested changes to be effective.
